Budget Impact 'Threshold'?

As we know, CET is not the only factor that affects reimbursement. One important factor is the budget impact. So, should we also have Budget Impact 'Threshold' as well, or at least some clarity on what kind of budget impact would likely be acceptable to the payer and/or government?
